Once you have scheduled an appointment with me, I will send you an Intake Packet. You can read it through and fill out the information prior to your first appointment, and we will discuss any questions you may have at that time. Please let me know if you are unable to print out the packet; you may then arrive 15 minutes early to fill out the paperwork.
